Hospicare Consulting is seeking a
Case Manager
to join our team in Santiago de los Caballeros. This is a
full-time, on-site position
focused on coordinating and overseeing client care plans while ensuring high-quality support services.

The Case Manager will work closely with clients, families, and healthcare professionals to assess needs, develop personalized care plans, and monitor progress, while maintaining accurate documentation and advocating for client well-being.

Key Responsibilities

• Assess client/patient needs and develop individualized care plans
• Coordinate and monitor multiple cases simultaneously
• Communicate effectively with clients, families, and healthcare providers
• Maintain detailed and accurate case records
• Advocate for client well-being and continuity of care
• Collaborate with multidisciplinary healthcare teams

Qualifications

• Experience in case management, client advocacy, or related roles
• Strong interpersonal, organizational, and time-management skills
• Knowledge of healthcare and social service systems
• Problem-solving, conflict resolution, and decision-making abilities
• Ability to work collaboratively with multidisciplinary teams
• Proficiency in Microsoft Office and case management software
• Fluency in
  Spanish and English
  (highly desirable)
• Bachelor's degree in Social Work, Nursing, or a related field
• Previous experience in healthcare or social services preferred
